Immigration Physical for Form I-693 for Green Card/US Citizenship

Are you applying for change of status or a Green Card?

When you come to the United States or if you are in the United States and are trying to adjust your status or seeking a safe haven or asylum – you many need to get a medical exam. The United States Citizenship and Immigration Services (USCIS) medical exam was put into place so that you can prove that you do not have any health conditions that would make you inadmissible to the United States. You should fill Form I-693. In order for your Form I-693 to be legitimate, a civil surgeon must approve and annotate your medical results on your physical form. Civil surgeons are accredited by USCIS to perform these examinations. Dr.Paul’s Clinic is approved by USCIS to provide these physicals. If you need one, please read the information on this page.

What to bring to your medical exam:

  1. Valid, government issued picture ID such as a valid passport, driver’s license, green card or military ID
  2. Medical records translated into English, including treatment for tuberculosis, if any
  3. Immunization/vaccine records translated into English
  4. Any Mental health records translated into English
  5. Drug abuse/addiction history, if any
  6. Any records of harmful behavior, if any
  7. List of previous and current medications, if you take any
  8. Previous tuberculosis screening, if any
  9. An immigration physical form I-693, pre completed with your information in CAPITAL LETTERS. Please use the most current form available on USCIS website https://www.uscis.gov/i-693. Then email it to us info@doctorpaul.org
  10. If you do not speak fluent English you must bring a translator with you

AFTER THE EXAM:

After the immigration medical examination is completed and all immunizations and lab tests have been performed, we will provide you with a certified form I-693 in a sealed envelope to present to the Department of Homeland Security and US Immigration Service. Do not open the sealed envelope. Turn in the sealed envelope with your immigration application. A copy of the blood test results and exam will be provided to you to keep after results have been discussed with Dr.Paul. Fees are required to be paid at the time of services.
